I the above parent/guardian have read Attitude Dance rules and regulations page. I fully understand and support these rules and I am happy for my child to attend weekly classes. I also will not hold Attitude Dance responsible in the unlikely event of any injury caused to my child.

I have also fully read and signed the data protection and Covid-19 form provided by Attitude Dance Studios LTD. Final Declaration

1. I certify that my child is physically fit and have not been advised against participating in exercise by a qualified medical professional, and that there are no health-related reasons precluding active participation in dance.

2. I will not hold Attitude Dance or instructors responsible for any unlikely injury/illness caused to my child.

3. I am not being forced to take my child to attend classes at Attitude Dance. This is my own / my child's decision.
